How to register

If you are new to the area we would be delighted to welcome you as a patient

Our practice list is open to people who live in the Grangetown, Riverside, Butetown,  Cardiff Bay and Cardiff City Centre areas (please see map below)

If you are unsure if you live within the practice boundary please ring, email or call into reception when the surgery is open to enquire about registration.

We are only obliged to register patients at our surgery who reside within our boundary.

Registering with the surgery

To register with the surgery you must complete the following forms:

  1. GMS1W GP registration Form
    Please print, complete, sign this form and return to the surgery
  2. New Patient Questionnaire
    Please print, complete all questions, sign this form and return to the surgery

Alternatively you can visit the surgery reception from 10am to 6pm and collect a paper versions of both forms.

In addition, all new patients should present two forms of identification (e.g. Passport or Drivers Licence and Council Tax Bill, TV Licence, or recent utility bill) which proves residency and authenticates their identity. This is required to reduce and prevent fraudulent attempts to obtain or misuse NHS care. In order to verify identity and ensure all paperwork has been  completed in the proper manner  these documents should bought to the practice in person along with the completed Registration Form.

Overseas visitors may be asked for additional information to prove NHS entitlement.
